I understand that I should seek medical advice from my general practitioner if I am in any doubt about my physical ability to take part in this event.  Sands will, in no way, be liable for any claim which may arise from this event. I acknowledge that the individual/organisation is undertaking this activity entirely of their own risk and that Sands (Stillbirth & Neonatal Death Society), shall not be liable for any injury or loss that might occur as a result of this event. I understand that Sands will, in no way, be liable for any claim that may arise form this event.
2. I agree to pay all proceeds of the event to Sands as quickly as possible after the event, no later than four weeks after.
3. By submitting this form I confirm that I am happy that Sands will use the details I have provided to fulfill my requests, to help me to help Sands deliver and improve the services for people affected by the death of a baby.  (Please read our Privacy Policy for more informaton on how we use your information.)
